The story begins with the Turtles celebrating their victory over the Shredder, who was seemingly killed by Splinter in the previous film. They enjoy their pizza and video games in their sewer lair, unaware that a new threat is brewing in the shadows. A mysterious figure, known as the Mutagen Man, has been collecting samples of the mutagen that transformed the Turtles and Splinter into their current forms. He plans to use it to create an army of mutant creatures that will obey his every command.
Meanwhile, April O’Neil, the Turtles’ human friend and reporter, is investigating a series of strange incidents involving animal attacks and disappearances in New York City. She discovers that the Mutagen Man is behind them, and that he has kidnapped her cameraman Vern Fenwick as a hostage. She manages to contact the Turtles and inform them of the situation, but is soon captured by the Mutagen Man’s henchmen.
The Turtles, along with Casey Jones, another human ally, rush to rescue April and Vern from the Mutagen Man’s secret laboratory. There, they encounter a horde of mutated animals, such as rats, dogs, cats, birds, snakes, and more. They also face some familiar foes, such as Baxter Stockman, a mad scientist who was turned into a fly by the mutagen, and Karai, the Shredder’s daughter who was mutated into a snake-woman. The Turtles fight their way through the lab, using their ninja skills and weapons to overcome the enemies.
They finally reach the Mutagen Man’s chamber, where they are shocked to discover his true identity: he is none other than Hamato Yoshi, Splinter’s long-lost brother and former leader of the Foot Clan. He reveals that he survived the fire that killed his family and scarred his face, and that he blamed Splinter for betraying him and joining the Hamato Clan. He also reveals that he was the one who revived the Shredder using the mutagen, and that he has been secretly manipulating him and the Foot Clan for his own agenda. He says that he intends to use the mutagen to create a new world order, where mutants will rule over humans.
The Turtles are enraged by Hamato Yoshi’s betrayal and madness, and challenge him to a final battle. Hamato Yoshi injects himself with a massive dose of mutagen, transforming into a monstrous hybrid of various animals. He proves to be a formidable opponent, as he unleashes his claws, fangs, wings, tentacles, and more against the Turtles. The Turtles struggle to fight back, but they are eventually overpowered by Hamato Yoshi’s brute strength and speed.
However, they are not alone in their fight. Splinter arrives at the scene, having sensed his brother’s presence through their psychic bond. He joins his sons in combatting Hamato Yoshi, hoping to reason with him and end his madness. He tells him that he still loves him as a brother, and that he never meant to hurt him or betray him. He pleads with him to stop his evil plan and join them as a family.
Hamato Yoshi is momentarily touched by Splinter’s words, but then rejects them as lies. He says that Splinter is weak and foolish for choosing peace over power. He says that he hates him for what he did to him and his family. He says that he will kill him and his sons, and then take over the world with his mutant army.
He lunges at Splinter with his claws, but Splinter dodges and counters with his tail. He knocks Hamato Yoshi off balance and pins him down with his weight. He looks into his eyes and says that he is sorry for everything that happened between them. He says that he hopes that one day they can be brothers again. He then delivers a fatal blow to Hamato Yoshi’s heart with his claws.
Hamato Yoshi gasps in pain and shock. He looks at Splinter with a mix of anger and sadness. He says that he hates him… but also loves him… as a brother. He then dies in Splinter’s arms.
Splinter mourns for his brother’s death, while the Turtles comfort him. They then free April and Vern from their cages, and escape from the lab before it explodes due to Hamato Yoshi’s self-destruct mechanism. They return to their sewer lair, where they celebrate their victory over Hamato Yoshi and his mutant army.
They also receive some good news from April: she has managed to expose Hamato Yoshi’s crimes to the public through her reportage, clearing the Turtles’ names from any suspicion or blame. She also reveals that she has found evidence that Hamato Yoshi was working with another mysterious figure, who was supplying him with advanced technology and resources for his plan.
The Turtles wonder who this figure could be, and what his or her motives are. They vow to find out and stop him or her, whatever it takes. They then toast to their friendship and family, and enjoy their pizza and video games. The film ends with a shot of the Turtles’ lair, where a TV screen shows the logo of the mysterious figure: a purple dragon.
